About LIFE

A Pennsylvania program providing comprehensive health and support services to seniors age 55+ who wish to continue living independently in their homes and communities

Eligibility

To qualify, you must be 55 or older, reside in a LIFE service area, and meet medical and financial requirements. Most Medicaid-eligible participants pay nothing for services.

Personal Care and In-Home Services

Home aides can assist with bathing, grooming, laundry, and light housekeeping. Home modifications like grab bars or ramps are also available based on your needs.

Medical Services

From primary care to specialists, prescriptions to therapies—LIFE coordinates all your medical needs. Doctors and nurses are on call 24/7 to help you.

Life Day Centers

Community centers where members can see doctors, receive therapy, enjoy free meals, and participate in fitness, recreational, and social activities with other LIFE members. Staffed by caring health professionals.

Transportation

Free transportation to the LIFE day center and all outside medical appointments, fully coordinated by your dedicated LIFE care team. No need to make calls or arrange rides on your own—LIFE handles it all for you.
